Pat-Med Pre-K Registration Starts April 10

Registration for pre-kindergarten children will run through May 31.

Contributed by the Patchogue-Medford School District.

 The Patchogue-Medford School District is accepting applications beginning Wednesday, April 10 through Friday, May 31, 2013 for pre-kindergarten children who qualify under the New York State guidelines.  

District requirements are: Birth certificate; physician’s verified proof of immunization; proof of residence (one of the following – these are the only documents accepted: Town/village property tax bill, closing statement/contract of sale, dully executed lease agreement [applicable only for apartment complex or condo association], notarized affidavit supplied by district for rentals in private homes, and utility bills (two) with parent’s name and address [no PO Box]); and proof of lead poisoning testing (if available).

Registration for children who are four years of age by December 1, 2013, and are eligible for kindergarten in September 2014, will be held at the Central Registration Office located in the Saxton Middle School, 121 Saxton Street, Patchogue, Monday through Friday between the hours of 9:00 a.m. and 12:00 p.m.  For information about registration, please call 687-6430.


Please note transportation is not provided for this program. The program will be held at Barton Elementary, Eagle Elementary, Canaan Elementary and River Elementary.  Each of the four sites will run a morning session and an afternoon session.  The program runs five days per week and follows the Patchogue-Medford approved 2013-2014 school calendar.  For further information about this program, go to www.pmschools.org or contact Assistant Superintendent for Instruction Lori Cannetti at 687-6350.
